Clinical Pharmacist - Manager of Provider Status Implementation

Job Summary:

The manager of provider status implementation will have a commitment to the advancement of excellence in the profession of pharmacy. This pharmacist will work to provide care and compassion with efficiency and optimization of medication therapy for the patients of Ziks Family Pharmacy and the Dayton, Ohio community. In addition this pharmacist will have responsibilities to develop opportunities and manage other pharmacy staff on provider status implementation efforts.

Requirements:

Ohio State Board of Pharmacy license to practice pharmacy. Preferred 3-5 years in clinical operations and or management of pharmacy. Strong organizational skills. Strong verbal and written communications skills.

Responsibilities:

  • Develop an effective and comprehensive approach to clinical patient care, including identification, monitoring, and management for complex patients
  • Optimize medication management for complex patients
  • Secure collaborative practice agreements with prescribers of Ziks Family
  • Pharmacy
  • Work with pharmacy ownership to develop clinical pharmacy goals
  • Develop advanced clinical services to meet pharmacy goals
  • Perform continuous quality improvement for the provider status programs at the pharmacy
  • Monitor Ohio-provider status initiatives to allow Ziks Family Pharmacy to quickly adapt and respond to pavement opportunities
  • Serve as an exemplary professional role model
  • Collaborate with community partners
  • Represent the pharmacy at medical or community meetings

Typical Functions:

  • Identify patients eligible for clinical services
  • Involve other pharmacists and pharmacy staff in provider status efforts
  • Participate in pharmacy planning and staff meetings
  • Perform efficient and accurate drug utilization review and management of the medication use system of the pharmacy
  • Perform sound one-on-one patient care with appropriate follow up
  • Provide regular formative and summative feedback to other pharmacist engaged in provider status initiatives.
